Business Licenses & Permits

To obtain a business license, click on the link below, fill out the application and bring to the Town Hall with your $25.00 application fee, or email the document to mayfieldtownutah@gmail.com and call us to pay over the phone for the application fee (additional fees apply). Once the application and fee are received, it will be forwarded to the Zoning Administrator for approval, once approved and any applicable license fee is paid, your business license will be sent to you.
